Intake Forms

Please fill out the Health History Intake Form included in your appointment confirmation email at least 12 hours in advance of your session. If the intake form is incomplete when you arrive, you will be asked to complete it in my office prior to treatment.  Please let me know if you require any accommodations or assistance in completing the forms.  


Cancellation/Rescheduling Policy 


24 hours notice is required to make any changes to your appointment.  As a courtesy, you will get a confirmation email 48 hours prior to your scheduled time. All changes, cancellations or rescheduling within 24 hours of your appointment or simply not showing up, will result in the full fee being charged.  Future appointments cannot be scheduled until the outstanding balance is paid.


Please note that frequent cancelling and rescheduling may result in terminating/discharging the therapeutic relationship.  


Sickness 

If you are sick, massage therapy is not going to make you feel better and could potentially make you feel worse. I know it can be frustrating to miss your appointment.  Allow your body the time it needs to heal and rest. Please note the 24-hour cancellation policy still applies in the event of illness or emergency.


Appointment Times

The appointment you have reserved is the length of time you have with me and begins as soon as we enter the treatment room. This includes reviewing your health history, assessments, treatment and follow up discussion regarding your experience during treatment. Out of respect and consideration of my time and other clients, please plan to be on time. If you arrive late, I will make the best use of your scheduled time and you will be responsible for the full cost of treatment. 


Open Communication

In order to serve your needs in the best possible way, I encourage you to communicate and share your needs, preferences, requests, feedback and questions at any time before, during, or after your appointment.  I truly want your honest experience (positive or negative).

Privacy Policy 

As a Regulated Health Professional, I adhere to the College of Massage Therapists of Ontario's Standards of Care regarding Privacy and Confidentiality If you would like further information or have concerns, please contact me so we can discuss it.  If you are unsatisfied with the resolution, you have the right to make a formal complaint to the Privacy Commissioner of Ontario within one year.
